National Advance Care Planning Week

ADVANCE CARE PLANNING WEEK 2018

National Advance Care Planning Week runs from the 16 - 22 April to raise awareness of advance care planning across Australia.


The week has been organised by Advance Care Planning Australia to encourage all Australians to start a conversation with their friends and family about what's important to them.

There are several ways to get involved including joining a local event.

The Heath Consumers Alliance and Office of the Public Advocate are both holding events for the national week.  For the full list of events CLICK HERE.


SA EVENTS

HEALTH CONSUMER ALLIANCE SA

    Community Forum
    When: 18 April 2018  4pm - 6pm
    Where: Science Exchange, 55 Exchange Place,
    Adelaide
    Keynote speaker: Ian Henschke

As part of the National Advance Care Planning Week the HCA, in partnership with Adelaide Primary Health Network and SA Health, will host Advance Care Directives (ACDs): A matter of love, rights and self-determination.

The event aims to increase awareness of the importance of ACDs in protecting individual rights, and encourage people to have the conversation and make their wishes clear in an ACD.

There will be four speaker presentations followed by a panel.

OFFICE OF THE PUBLIC ADVOCATE



Community Education Session
When: 16 April 2018 - 10:30am - 12:30pm (please arrive 10-15 minutes early)
Where: Level 7, ABC Building, 85 North East Road Collinswood. Please arrive 15 minutes earlier


The Office of the Public Advocate will be providing a free consumer education session: a Pathway to Your Future Care.

This will cover all the relevant information you need to put your future plans in place in case you become unable to make your own decisions.

The session will provide detailed information about Advance Care Directives and Enduring Powers of Attorney, including what they are, how to complete one and how they will be used.

There will be Q&A session and free Advance Care Directive forms, Do It Yourself Kits and brochures will be available for all participants.

Tea and coffee will be provided, so we hope to see you there.
